It doesn't always require changing the story substantially, especially if the story was AU to begin with -- those kinda seem tailor-made for being halfway to ofic already. The problem, I think (and based on my own experience of attempting such), is that the fanfic-author can't separate the original emotional context of the fandom from the story itself. So you have parts, like the piano-playing, that got such a huge reaction from the original readers (and is probably a fairly fondly-considered moment, in the author's eyes) and yet doesn't work if the story is ofic. It's entirely there for emotional reasons, making it really tough to be surgical enough to excise the scene, let alone to even see that it doesn't belong at all.
