Hmmm. Maybe a mishap, not with nuclear weapons, but with bio weapons? That's the kind of thing where one stupid little mistake really could have horrible, wide-ranging, long-lasting effects. And also a situation where the nations/governments involved might feel a moral obligation to care for the victims, no matter what the strain on resources. And where the ability to have undamaged children could be vital and needed at the same time that reproduction still exceeds the death rate; it could also explain a reluctance to terminate damaged children, since they are seen as victims, too.
This would require almost worldwide first world lifestyles before the crash, I think. No one else has quite the cultural dedication to altruism as a nation whose stomachs are, by and large, filled already.
